Sweos.org Wellbeing Hub service is the hub service and support Somali community in different ages and abilities with their Health and Social care communication and Wellbeing support needs.
Wellbeing Hub service is the hub service and support Somali community in different ages and abilities with their providing holistic approach focussing on individual need. Promoting health and wellbeing that reduces health inequalities in our community setting, using nonclinical methods. Addressing barriers of communication and Wellbeing support they need to make their lives better.
Wellbeing & Healthy Weight.
We will support and expand our healthy weight programme by ensuring that people on low incomes are able to access healthy weight. We will expand and collaborate with local Health Groups to build opportunities for community-based physical activity for older people, linking with the volunteering and empowering communities.
Reduction in the prevalence of unhealthy weight in the adult We will commission a weight management service including a focus on enhancing uptake among those in need to reduce weight. We will review our Health Checks programme and explore opportunities to maximise uptake of Health ensuring a focus on those at highest risk.
We will record it weekly and check clients before and after BMI / weight, Blood Pressure and achievement of weekly goals, pre and post programme evaluation.
The service will be subject to a comprehensive evaluation process where all clients are telephoned 3 months after they have accessed the service and these outcomes will feed into the review reports and service improvements We worked with some organisations like Health watch Harrow and. Central London Community Health Care NHS and will be looking for ways to improve engagement with a range of other organisations. We will continue to deliver the new pre diabetes program where people at risk of type 2 diabetes can understand how to prevent the disease through their diet and with regular exercise.
The service will be subject to a comprehensive evaluation process where all clients are telephoned to support people with multiple, complex health needs who depend on many health and social care services to meet those needs. We will work closely with other parts of the health and care system, such as GPs, hospitals, pharmacies. The increasing numbers of people living with long-term in Somali Community means that more people are likely need support from community health services and supporting community to be connected with other professionals.
